Here are the details of the benefits program at Primary Capital for full-time employees. Medical benefits coverage is effective on the 1st of the month following 30 days of employment.

Medical : Humana will provide Primary Capital’s comprehensive medical coverage. Georgia employees may choose from two HMO plans, POS or the HSA plan (referrals to specialists are not required with either of the HMO plans). Out of state employees may choose the POS or the HSA plan. The co-pays, deductibles, and the semi-monthly premium you are responsible for paying will differ according to the plan that you choose and the number of eligible covered dependents. Primary Capital’s medical benefit also provides prescription benefits as well as a mail order option for prescription maintenance drugs. Dental : Dental coverage is administered through UCCI. With this plan, you are free to choose the dentist of your choice or choose one from their list. Preventative services are covered at 100% of reasonable and customary charges and include up to two visits per year, per covered individual. The annual deductible is $50 for each covered individual per calendar year for basic or major services. Basic and major services are covered at 80% and 50% respectively. The annual maximum benefit is $2,000 per covered individual. Orthodontic coverage is available for dependents under 18 years of age (50% benefit and a lifetime maximum of $1,500). Vision : Vision coverage is provided through Humana Vision Care. In-network coverage includes an annual eye exam for a $10 co-pay. There is also an allowance for frames or contact lenses every 12 months. You must go to a provider on their list. Employee Assistance Plan : You and/or any of your immediate family members are eligible (at no cost to you) for up to six visits per calendar year to provide you assistance with marriage and family concerns, interpersonal difficulties, stress management, etc. This benefit also offers 30-minutes of legal advice per calendar year at no cost to you. Flexible Spending Account (FSA) : This benefit provides tax advantages for certain types of health and dependent care expenses. Your FSA may be used to reimburse out-of-pocket medical, dental, and vision care expenses, or dependent care expenses that are defined as eligible by the Internal Revenue Code.
