full-time employees. Medical benefits coverage is effective on the
1st of the month following a new employee's date of employment.
Medical : Blue Cross Blue Shield will provide Primary
Capital’s comprehensive medical coverage. You may choose from
two HMO plans or the PPO plan (referrals to specialists are not required
with either of the HMO plans). The co-pays, deductibles, and the semi-monthly
premium you are responsible for paying will differ according to the
plan that you choose and the number of eligible covered dependents.
Primary Capital’s medical benefit also provides prescription
benefits as well as a mail order option for prescription maintenance

Dental : Dental coverage is administered through
Met Life. With this plan, you are free to choose the dentist of your
choice or choose one from their list. Preventative services are covered
at 100% of reasonable and customary charges and include up to two
visits per year, per covered individual. The annual deductible is
$50 for each covered individual per calendar year for basic or major
services. Basic and major services are covered at 80% and 50% respectively.
The annual maximum benefit is $2,000 per covered individual. Orthodontic
coverage is available for dependents under 18 years of age (50% benefit

Vision : Vision coverage is provided through EyeMed
Vision Care. In-network coverage includes an annual eye exam for a
$10 co-pay. There is also an allowance for frames or contact lenses

Employee Assistance Plan : You and/or any of your
immediate family members are eligible (at no cost to you) for up to
six visits per calendar year to provide you assistance with marriage

Flexible Spending Account (FSA) : This benefit provides
tax advantages for certain types of health and dependent care expenses.
