While no one likes to think the future may bring catastrophe, it is always best to plan for the worst. We have several programs to assist our employees in making sure they and their loved ones are cared for in any event.

Short-term Disability : This benefit provides income for you when you are absent from work due to a non-occupational illness, injury, or pregnancy-related disability up to 90 days. After using your accrued sick leave and a seven day waiting period and completing one year of service you will be paid as follows:

6 or more	7 days	75%

Long-term Disability : Primary Capital provides you with a long-term disability benefit in the amount of 60% of your annual earnings to a maximum of $10,000/month. This plan is effective after a 90-day elimination period and until the doctor releases you to return to work. You are also eligible to purchase Supplemental Income Protection that would increase your monthly disability payments to approximately 75% of your monthly income.

Cancer Insurance : This coverage is provided by ALFAC and you may choose coverage for yourself, as well as your spouse and children. The cancer insurance pays cash benefits directly to you upon initial treatment allowing you the freedom to use the funds as you see fit. Optional benefit riders are available.

Accident Insurance : This coverage is provided by ALFAC and you may choose coverage for yourself, as well as your spouse and children. The accident coverage provides you with an accident emergency room treatment reimbursement benefit of $120, an accident follow-up benefit of $35 per visit for up to six visits, and an accident hospitalization benefit of $1,000 when a covered individual is confined to a hospital for at least 24 hours for injuries sustained in a covered accident.

Life Insurance : Primary Capital provides you with Life Insurance (with an additional Accidental Death and Dismemberment rider) at two times your annual income (up to a $300,000 maximum) at no cost to you. You also have the option of purchasing Supplemental Life Insurance equal to up to five times your annual earnings.There is also optional Dependent Life Insurance available.

Long-Term Care Insurance : This coverage is provided by Prudential and you are able to customize your plan coverage based upon your needs. Long-Term care provides a wide range of medical and support services for people with a degenerative condition, a prolonged illness or cognitive disorder.
